This form asks for personal data about your and your family, birth and marriage dates, and your financial and asset information. To provide you with a thorough evaluation and recommendation for estate planning, you must provide our attorneys with a complete understanding of what you own, with whom you own it, in what jurisdiction you own it, and its value. Please be as complete as possible about those elements so that we may accurately advise you.
